  • Valid passports for both parties
  • Notarized single status affidavit for each
  • Address & occupations of both parties who will be married
  • Names of parents & their addresses
  • Names of grandparents
  • Tourist card
  • Two witnesses (not family), their names, addresses, & valid passports
